Please be prepared to upload all the paperwork required with the Lack of Form:
- Recent Baptismal Certificate for the Catholic party in this union.
Be sure that the certificate was issued within the last six months.
The Baptismal Certificate must indicate “notations” or “no notations” regarding any previous marriage.
- Certified copy of the Marriage Certificate; (unless it is Common Law)
- Certified copy of the Divorce / Dissolution / Civil Annulment decree
- $50.00 Processing Fee – paid by credit card
- Approval from you, as Advocate, is required
